To reach the on-call pediatrician after hours (for emergencies that cannot wait until the office opens in the morning):

Call the answering service: (866) 538-4542. If you do not receive a call back within an hour or so, check again with the answering service to ensure that they have your correct phone number.

If you wish to transfer your children’s pediatric care to me at my new practice, you will need to complete a Request to Transfer Records form, and mail, fax, or hand deliver the completed transfer authorization to:

All charges usually associated with records transfer will be waived.

If you do request a transfer your child’s medical records will automatically remain with Pediatric Associates, P.A. and they will continue to provide care for your child.
